Fees, Forms, & Insurance



 
                                 Insurance & Fees

                      My current rate for an individual session is $175. 
                      Groups that meet every other week are $60 per session.
                    Groups that meet monthly are $80 per session. (Two virtual consultation sessions are required before joining the group; these sessions may be insurance reimbursable.)
                            

Please note that your actual group experience does not meet the medical-necessity criteria for insurance coverage. My groups are designed to support self-awareness, personal growth, and greater life satisfaction; they do not provide treatment for mental illness.

I accept payment by check or by Zelle (secure bank-to-bank transfer).
Payment is due at the beginning of each session unless we have made other arrangements.

Cancellation policy for individual sessions:
Cancellations require 48 business hours’ notice to avoid being charged the full fee.

For Monday appointments, cancellations must be made by the end of the business day on the preceding Thursday.
   Please note that insurance does not cover or reimburse missed sessions; any fees for appointments canceled with less than 48 business hours’ notice are your responsibility and are handled outside of insurance billing.         
             
      I am a Preferred Provider for Lyra EAP.   
You can check if your employer contracts with Lyra here or contact your company’s Human Resources Department.

      As a Preferred Provider for Cigna, I’ll bill your insurer directly, and you’ll owe me your copay/coinsurance (and deductible if it isn’t paid yet).  It’s always a good idea to check with your insurer regarding  coverage specifics. If your plan requires pre-authorization, it is your responsibility to acquire authorization.

      I am an Out-of-Network Provider for all other health care plans.    
 
     As an Out-of-Network provider, you’ll pay me up front, and I’ll provide you with the necessary form ("Superbill") to submit for reimbursement As an out-of-network provider my agreement is with you, rather than with your insurance company.    It’s always a good idea to check with your insurer regarding coverage specifics.

I am not a Medicare provider;
this means I am not able to provide coverage through Medicare supplemental plans either.

     You may choose not to use your health insurance for therapy to maintain a higher level of confidentiality.  When a health insurance plan isn’t involved, the work is simply between you and me with no third party involvement. 


"Good Faith Estimates"
                        As of January 1, 2022 you are entitled to a good faith estimate of your expected costs of service.                                   I have provided the fee information above in order to make the cost of service as transparent as possible. 
You will also be provided with a written Good Faith Estimate specific to you at the time of scheduling.
