Fees, Forms, & Insurance




 
                                   Insurance & Fees

                      My current rate for an individual session is $150-175.
                      My current rate for groups is $55-$80 per session.
                            
                            (*Please note that my groups do not meet the medical necessity criteria to be
                            covered by insurance.  These groups are designed to increase self-awareness and life satisfaction, and do not treat mental illness.

                     I accept payment by check, cash, or Zelle secure bank-to-bank transfers.
​       Payment is due at the beginning of each session unless you & I have another agreement.
                     
​                     Cancellation policy for individual sessions: 48 hours’ notice to avoid being charged full fee;
                            Monday appointments need to be canceled by the end of the business day on the previous Thursday.
             
      I am a Preferred Provider (in-network) for the following health care plan:

      As a Preferred Provider, I’ll bill your insurer directly, and you’ll owe me your copay/coinsurance (and deductible if it isn’t paid yet).  It’s always a good idea to check with your insurer regarding  coverage specifics. If your plan requires pre-authorization, it is your responsibility to acquire authorization.

      I am an Out-of-Network Provider for all other health care plans:

    *Premera, Anthem & Lifewise

  *First Choice Health
 
     As an Out-of-Network provider, you’ll pay me up front, and I’ll provide you with the necessary form to submit for reimbursement. As an out-of-network provider my agreement is with you, rather than with your insurance company.    It’s always a good idea to check with your insurer regarding coverage specifics.

I am not a Medicare provider;
this means I am not able to provide coverage through Medicare supplemental plans either.

     You may choose not to use your health insurance for therapy to maintain a higher level of confidentiality.  When a health insurance plan isn’t involved, the work is simply between you and me with no third party involvement. 


"Good Faith Estimates"
                        As of January 1, 2022 you are entitled to a good faith estimate of your expected costs of service.                                   I have provided the fee information above in order to make the cost of service as transparent as possible. 
You will also be provided with a written Good Faith Estimate specific to you at the time of scheduling.
